Ishys, a takeaway at 105 Oak Road, Partington, Manchester was given the score after assessment on September 13, the Food Standards Agency's website shows.
It means that of Trafford's 264 takeaways with ratings, 156 (59%) have ratings of five and just one has a zero rating.
