Trafford Oriental, a takeaway at 18 Seymour Grove, Old Trafford, Manchester was given the score after assessment on July 20, the Food Standards Agency's website shows.
It means that of Trafford's 259 takeaways with ratings, 126 (49%) have ratings of five and just one has a zero rating.
