Andertons Fish & Chips, a takeaway at 2 St James Road, Salford was given the score after assessment on September 14, the Food Standards Agency's website shows.
It means that of Salford's 318 takeaways with ratings, 106 (33%) have ratings of five and none have zero ratings.
