Cafe Aylanto, a takeaway at 520 Bury Road, Rochdale, Lancashire was given the score after assessment on October 14, the Food Standards Agency's website shows.
It means that of Rochdale's 291 takeaways with ratings, 156 (54%) have ratings of five and just one has a zero rating.
