China Chef, a takeaway at 829 Manchester Road, Rochdale, Lancashire was given the score after assessment on September 23, the Food Standards Agency's website shows.
It means that of Rochdale's 292 takeaways with ratings, 154 (53%) have ratings of five and just one has a zero rating.
