LA Pizza, a takeaway at 34 Market Place, Heywood, Lancashire was given the score after assessment on August 8, the Food Standards Agency's website shows.
It means that of Rochdale's 293 takeaways with ratings, 150 (51%) have ratings of five and just one has a zero rating.
