Istanblue, a takeaway at 37 Milkstone Road, Rochdale, Lancashire was given the score after assessment on September 15, the Food Standards Agency's website shows.
It means that of Rochdale's 293 takeaways with ratings, 154 (53%) have ratings of five and just one has a zero rating.
