Kamran Sweet Centre, a takeaway at 59-61 Milkstone Road, Rochdale, Lancashire was given the score after assessment on September 12, the Food Standards Agency's website shows.
It means that of Rochdale's 293 takeaways with ratings, 153 (52%) have ratings of five and just one has a zero rating.
