My Peri Chicken, a takeaway at 170b Spotland Road, Rochdale, Lancashire was given the score after assessment on September 12, the Food Standards Agency's website shows.
It means that of Rochdale's 293 takeaways with ratings, 151 (52%) have ratings of five and just one has a zero rating.
