The Second Serving, a takeaway at 503 Oldham Road, Rochdale, Lancashire was given the score after assessment on July 22, the Food Standards Agency's website shows.
It means that of Rochdale's 292 takeaways with ratings, 150 (51%) have ratings of five and just one has a zero rating.
