Unit 11, a takeaway at Unit 11 Riverside Walk, Rochdale was given the score after assessment on July 21, the Food Standards Agency's website shows.
It means that of Rochdale's 291 takeaways with ratings, 150 (52%) have ratings of five and just one has a zero rating.
