Failsworth Tandoori and Pizza, a takeaway at 487 Manchester Road, Oldham was given the score after assessment on September 20, the Food Standards Agency's website shows.
It means that of Oldham's 274 takeaways with ratings, 146 (53%) have ratings of five and just two have zero ratings.
