Nashmi Grill, a takeaway at 149 Lloyd Street South, Manchester was given the score after assessment on October 31, the Food Standards Agency's website shows.
It means that of Manchester's 640 takeaways with ratings, 235 (37%) have ratings of five and just eight have zero ratings.
