Lazeeza Takeaway, a takeaway at 592 Stockport Road, Manchester was given the score after assessment on September 16, the Food Standards Agency's website shows.
It means that of Manchester's 611 takeaways with ratings, 207 (34%) have ratings of five and just seven have zero ratings.
