Tasty Grill, a takeaway at 3 Gilnow Road, Bolton was given the score after assessment on June 25, the Food Standards Agency's website shows.
It means that of Bolton's 380 takeaways with ratings, 121 (32%) have ratings of five and just five have zero ratings.
