Curry Centre, a takeaway at 68 Egerton Street, Farnworth, Bolton was given the score after assessment on May 12, the Food Standards Agency's website shows.
It means that of Bolton's 378 takeaways with ratings, 117 (31%) have ratings of five and just five have zero ratings.
